PATIENTS
PROVIDERS

There is increased awareness of the importance of Patient Reported Outcomes (PRO) in the treatment of chronic conditions like cancer and HIV infection.  BrightOutcome brings home-based patient reporting into the clinical care setting systematically to benefit both clinician and patient. Our Collaborative Symptom Management helps to improve physical functioning, reduce suffering, and avoid unnecessary clinic/ER visits.

The BrightOutcome solution
BrightOutcome uses the latest patient-reported outcomes (PRO) research and e-health/telehealth technologies to conduct adaptive PRO assessments anywhere and any time.

Online, on the phone or using a PDA, patients respond to symptom survey questions that “adapt” so that the next question relates to the prior answer. The patient input goes into the provider’s work flow system, builds interactive care management task lists, and creates summary charts for provider and patient to view together. Emergencies trigger special alerts, on a nurse’s Blackberry as an example. Patients are linked to educational content relevant to their specific symptoms and concerns.

Patients are empowered to self-manage their symptoms and communicate more effectively with care providers. Clinicians are able to provide personalized care to patients and make better decisions. Quality time and clear communication increases mutual trust and self-efficacy, and reduces costly crises such as emergency room visits that could have been proactively avoided.

Circle of careConnecting everyone in the circle of care
Patient-centric care requires participation from many sources, working together. BrightOutcome is the web-based hub for everyone to share information and track treatment progress. Patient reports and clinical care history are integrated into the same real-time database. Patients are linked to relevant sites providing knowledge to discuss with caregivers, support groups and family as well as physicians.
